      Overview

      The Social Worker's primary purpose is to provide services to meet the social and/or emotional needs that affect the residents' ability to achieve their highest level of function; participate in the development of residents' comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state, and local regulations.

      Responsibilities

      • Explains residents' rights to residents and residents' representatives
      • Develops/coordinates family and resident activities designed to promote social interaction, reality orientation, and intellectual stimulation
      • Develops one-to-one professional relationships with residents and families as needed for counseling
      • Provides resources for residents and families in dealing with feelings about death or dying and other emotional, mental, environmental, or physical limitations
      • Assesses and completes the assigned section(s) of the Minimum Data Set (MDS) completely, timely, and accurately
      • Documents the social service component of the Comprehensive Care Plan for each resident in a timely manner
      • Documents psycho-social needs, interactions, and follow-up action(s) with resident and/or family according to facility policies and procedures
      • Acts as liaison with social, health, and community agencies
      • Refers residents to social, health, and community agencies and completes accurate documentation in residents' records concerning the results of such referrals
      • Coordinates family and community resources as needed to solve financial needs and to promote emotional security
      • Coordinates and cooperates with the Activity Department to plan and conduct events to meet socialization needs of the residents
      • Takes part in The Caring House in-service programs as required
      • Assists in the admission and discharge process of residents as required.
      • Displays tact and sound judgment in dealing with varying situations involving physicians, residents and their families, and visitors, as well as management and staff

      Qualifications

      • Required Qualifications (Behavioral Health):
      • Master’s Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE).
      • Current and valid State of Arizona license as a Social Worker.
      • 2 years of experience in Case Management reuired.
      • Current and continuing CPR and AED training and certifications.
      • AZ Department of Public Safety Fingerprint Clearance Card.
      • State of AZ Driver’s License with no significant violations during the last 5 years.
      • Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
      • Required Qualifications (Non- Behavioral Health):
      • Master’s Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE).
      • Current and continuing CPR and AED training and certifications.
      • AZ Department of Public Safety Fingerprint Clearance Card.
      • State of AZ Driver’s License with no significant violations during the last 5 years.
      • Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
      • Required Qualifications (The Caring House):
      • Minimum Bachelor’s degree in social work; OR Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education. Master’s degree in social work is preferred.
      • Minimum of one year experience in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals.
      • Current and continuing CPR and AED training and certifications.
      • AZ Department of Public Safety Fingerprint Clearance Card.
      • Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
